Clausen House: Independent Living Services Program

Our Independent living services program offers person-centered training and assistance based on the preferences, personality and hopes for the future that each client we have the privilege to serve brings with them. We honor all the unique qualities that make each client the incredible one of a kind person that they are! Services and support are designed to allow the individuals we serve to live as independently as possible in homes of their own in the community.

A thorough assessment is completed prior to beginning service delivery to identify areas of strengths and needs so that a truly individualized service plan can be created along with the client that respects their individuality as well as seeks to assist them in reaching their fullest potential.

Areas of functional living training and assistance that are typically covered but are not limited to are:

  • Money Management
  • Access to Healthcare/Medical self-management
  • Nutrition/Meal Preparation
  • Apartment Maintenance
  • Community Integration/Safety Awareness
  • Self-advocacy
  • Problem solving
  • Emergency/Disaster Preparation

Clients may receive up to 40 hours per month of training and assistance with the expectation that the number of service hours may diminish over time as the client’s level of functioning independently grows. Ongoing support typically continues at a lessor rate of 15 hours per month to ensure that the progress made towards successfully and safely living independently is maintained for the long term. A 24hr emergency response paging system allows clients to have access to support 24 hours a day 365 days a year in the event of an emergency.

Clausen House: Supportive Living Services Program

Our Supported living services program provides support and instruction to adults with intellectual and developmental disabilities living in a home of their own in the community. A client may receive a significantly higher number of hours of support and instruction in our Supported living services program versus our Independent living services program based upon their need, however services are also delivered with the same person-centered approach and an individualized service plan is designed along with the client which values their unique personality, preferences and goals.

Supported living services are typically more intensive and can include but are not limited to:

  • Personal assistance
  • Facilitation of the use of adaptive equipment
  • Shopping
  • Meal Preparation
  • Healthcare/medication management
  • Apartment maintenance
  • Community Integration/Safety Awareness
  • Disaster/Emergency Preparation

A thorough assessment is also completed prior to beginning services, however unlike independent living services the number of hours of service are not expected to decrease over time. Services and supports are provided on an ongoing basis to allow the client to live safely and comfortably in a home of their own in the community for the long term. A 24hr emergency response paging system allows clients to have access to support 24 hours a day 365 days a year in the event of an emergency.
